An application for a safety confirmation test
The manufacturer or importer of the products subject to safety confirmation requirements shall have each model of her products tested, with the following documents submitted along with the application. If a manufacturer that is located overseas wishes to have a safety confirmation test, it is possible to have an agent in South Korea to file the safety confirmation report on her behalf. When requesting a safety certification body to get a safety confirmation test, it is possible to submit the safety confirmation report at the same time.

- A copy of the business registration certificate or equivalent documents (if there is more than one factory that manufactures the same product, the document shall be submitted for each of these factories.)
- A power of attorney (if the application is submitted by an agent)
- Product manual
- Electrical circuit diagrams and lists of parts (only if it is an electrical product that requires safety confirmation).
- The shop drawings for the mechanical and electrical designs and other technical documents (only for those applicable to Item 3, Paragraph C, Section 2 of Table 2)
- The safety confirmation test report from a safety confirmation test agency
- The ingredients and mixing ratio of the product (only for the products in Items 2) and 4) under Paragraph A, Section 2 of Table 4, including the items of the same model with the product of the application but of different ingredients.)
- This applies only to the materials that are included in the products of application (Items 2) and 4) under Paragraph A, Section 2 of Table 4), including the cases where the models are the same, but the ingredients are different.
- Documentary proofs of identical products (only applicable when there is more than one factory that makes the same product.)
- A sample of safety confirmation according to Paragraph 1, Item 1 under Article 18 of the law
If an importer or a manufacturer of imported automobiles requested a safety certificate in the first place, then, for the request of issuance of a sample certificate of the foreign manufacturer to clear the customs for conducting the test, have a safety certification exemption certificate from Korea Product Safety Association.
